New Horizons 24/7 Recruitment works as a step down service from hospital to the community, providing
support
for service users who are ready for discharge but not yet ready to live independently due to lack of personal skills and daily living. This will help the hospital to release beds for other service users and this will assist service users to get on with life without relapsing due to staying longer in hospitals, unnecessarily.
The services also include
respite care
for adults, giving carers a chance to relax and cope with caring for loved ones.
Our clients are referred to us by hospitals, NHS trusts, GPs and private arrangements through families. These can be funded by private arrangements or direct payments from the NHS trust (commissioners).
New Horizons 24/7 Recruitment will be sanctioned by the CQC (Care Quality Commission) and will be working in collaboration with:
- NHS trusts (Commissioners) – This could be on a nationwide basis as individuals are often displaced due to a shortage of beds. A meeting was held locally with commissioners in Teesside and they are willing to use our services and are waiting for us to be up and running. They want a site visit.
- Local authorities – (This could be nationwide as the individual home teams are prepared to pay for the agreed package)
