New Horizons 24/7 Care Services is based in Stockton on Tees and are offering a step-down service in the North of England to service users who are ready to be discharged out of hospital, requiring support in the community.
New Horizons 24/7 is a recovery focused service based on the recovery model, offering bespoke packages of care supporting service users in their own tenancy.
Our model of care is to continue care when service users are well enough to be discharged, however, still need certain level of care and support in order to sustain their recovery.
The Individual bespoke packages of community care aims to support service users with social inclusion and smooth integration into the community.
Our care packages range from several hours per day up to 24/7 support in their own homes, giving the service users and carers a chance of social inclusion and a better quality of life away from the restriction of hospital regime. We also offer a respite care service.
New Horizons 24/7 is currently working with the Commissioners in Hartlepool where they are offering a 24/7 care package including sleep-over nights.
We offer a 24 hour on call RMN cover service.

New Horizon 24/7 is a nurse led company. It is managed by Nurses with more than 20 years’ experience working in mental health, Learning Disabilities and General Nursing.
New Horizon’s 24/7 achieved a good rated service in all areas inspected by The CQC in November 2018.
New Horizons 24/7 Recruitment offers 24/7 services for service users with mental health and learning difficulties, and physical disabilities. We offer interim Integrated Care’s Supported Living services to adults and young people, of all ages and abilities, equipping them with the skills and the opportunity to move forward towards living independently in their own homes.
